私の感覚だと、「自分が決めた」時など、誰が決めたのかはっきりしているとき、「決める」という動作にfocusをするときには決定を使うことが多い気がします。
例えば、「私たちは、旅行の日程を決定する」

確定は「決まった状態」にfocusをしていることが多いような気はします。誰が決めたとかは、あまり重要ではない、「決まる」に近いような気がします。
例えば、「旅行の日程が確定する」「このボタンをクリックすると、注文が確定します」

つまり
「決定する」＝「決める」（transitive）　
「確定する」＝「決まる」（intransitive）
のように使われることが「多い」とは思います。（多い・・・と思うだけで、「必ず」ではありませんので、注意してくださいね :wink: ）
